376 - How can I Coordinate care with the ED and the Hospital

Session Description: You may have clients who need intensive induction onto MAT or who are having severe withdrawal. Perhaps you have a client about to undergo surgery and you want to ensure that your plan of care is followed while they are in the hospital. There may be clients who are hospitalized for complications of SUD and you want to insure a smooth transition of care between the inpatient stay and your practice. The purpose of this session is to outline ways to work with the emergency department (ED) providers and the hospitalists to insure that occurs. How can you best utilize the resources available in the ED and the hospital when the client has needs that exceed what you can provide on an outpatient basis? This workshop will outline methods of engaging the hospital providers, demonstrating successful programs and pitfalls that were seen during implementation of those programs. They included MAT induction for complex SUD patients, management of complicated withdrawal syndromes, maintenance of MAT for patients undergoing surgery, and initiation of MAT with transition to the outpatient practice. Protocols and techniques include helping their clients with difficult induction or withdrawal, ensure appropriate treatment for clients undergoing surgery, and offering a smooth transition for patients discharged from the hospital after admissions that involve SUD issues. The attendee will be able to advocate for appropriate resources to manage clients in the ED and hospital environment.
